We are pleased to announce that Claremont Meadows Medical Centre now offers Home Medication Reviews. This service is available to individuals who are taking five or more types of regular medications daily. As a government-funded program, it is provided to you entirely free of charge.
What are the benefits of the Home Medication Review? Our accredited pharmacists will:
- Educate you on proper medication usage
- Review your current medications
- Assess your techniques for using inhalers or injections at home
- Explain the advantages and disadvantages of your medications
- Provide a comprehensive medication review report for your GP’s consideration
How often can you utilise this service? You are eligible to request this service once a year. Your GP can assist in setting up a reminder for your next review, ensuring you receive a timely SMS notification.
What are the costs involved? This service is completely free for eligible individuals, thanks to government funding.
How does it work and what can you expect? A pharmacist will contact you to schedule a convenient time for a home visit, during which they will assess your medication regimen in the comfort of your own home.
